Our office is conveniently located at 906 Business Park Drive, accessible off Three Mile Road just south of US-31 North.  The entrance to Business park drive is on the East side of the road, just opposite Aero Park Drive. Grand Traverse Family Practice is approximately 1/4 mile down on the right hand (South) side.

Dr. Swan holds office hours on Tuesday, Thursday & Friday from 8 AM till 5 PM by appointment.  Special hours can be arranged if necessary.  To schedule an appointment, please call  (231) 933-6600.  The office is always open with cross coverage provided by Kay VanderRoest, PA-C and Greg Hessler, M.D.
